Description
Arising in the soft tissues of the mental and submental region, the submental vein tracks posteriorly and slightly inferiorly beneath the mandibular symphysis, coursing superficial to the mylohyoid and in close relationship to the anterior belly of the digastric. Small venules from the chin and lower lip converge medially, then drain laterally toward the submandibular triangle where the vessel typically joins the facial vein near the inferior border of the mandible. Nearby, the mandibular body and angle provide bony landmarks, while the masseter sits posterolateral to the oral commissure and the parotid and submandibular glands occupy their expected lateral and inferomedial positions. Nerve branches in the region include the marginal mandibular branch of the facial nerve running anterior to the masseter and the mental nerve emerging from the mental foramen. A lateral perspective clarifies the layered anatomy that matters in submental and submandibular procedures. During submental liposuction, platysmaplasty, or open approaches for mandibular symphysis fractures, the submental vein can be a source of brisk venous bleeding and postoperative hematoma when it is avulsed near its confluence with the facial vein. Nerve safety is paired with hemostasis here, since the marginal mandibular nerve often lies in the same operative corridor along the inferior mandibular border. Small space, high consequence.
