Description
Posterior to the larynx and proximal trachea, the right and left thyroid lobes flank the midline isthmus while the parathyroid glands sit on the posterior surfaces, typically as superior and inferior pairs. From this posterior perspective, the superior thyroid arteries descend toward the upper poles of the gland, tracking inferomedially from the carotid region and fanning into capsular branches along the posterior aspect of each lobe. The vessels lie close to the pharyngeal wall superiorly, with the cricothyroid region and upper tracheoesophageal groove forming key deep landmarks. Small-caliber arterioles spread across the lobular gland surface. Tight anatomy. That posterior relationship matters in thyroid surgery because the vascular pedicle at the superior pole is where hemostasis and nerve safety collide. Ligation of the superior thyroid artery too proximally risks injury to the external branch of the superior laryngeal nerve as it courses near the superior thyroid vessels en route to the cricothyroid muscle, a setup for postoperative voice fatigue and loss of high pitch.
