Running transversely across the anterior aspect of the tibial plateau, the transverse ligament of the knee links the anterior horn of the medial meniscus to the anterior horn of the lateral meniscus. From an anterior perspective you also expect the patella and patellar ligament to sit anterior to the femorotibial joint line, with the femoral condyles superior and the tibial condyles inferior, framing the meniscal fibrocartilage on the medial and lateral sides. Collateral ligaments lie more peripherally, with the fibular (lateral) collateral ligament positioned lateral to the joint capsule and the tibial (medial) collateral ligament more medial and capsular. This anterior view is the one that clarifies how a small, easily overlooked band can couple anterior meniscal motion during early flexion and extension, and why it can be mistaken for, or confused with, adjacent anterior intermeniscal fibers on arthroscopy. It matters in meniscal surgery: during repair of an anterior horn tear or during meniscal transplantation, inadvertent sectioning or misidentification of the transverse ligament can alter anterior horn positioning and contribute to postoperative meniscal extrusion.
