Description
Running along the posterior aspect of the brachium, the paired deep collecting veins (venae comitantes) track closely with the brachial artery, flanking it as they course superiorly along the humeral shaft toward the axilla. Proximally, these channels converge into the axillary vein at the inferior border of teres major, then continue as the subclavian vein beneath the clavicle into the central venous system of the upper thorax. Distally, the deep venous drainage arises from the forearm venae comitantes and interosseous veins, passing proximally through the cubital region before aligning with the neurovascular bundle in the arm. Bony context is implied by the humerus as a longitudinal landmark, with the venous pathway remaining medial to the deltoid mass and posterior to the anterior compartment musculature. Posterior emphasis on the deep venous system matters when you need to teach or plan around structures that are not apparent in superficial venous maps, particularly during evaluation of upper extremity deep vein thrombosis or when tracking the proximal extent of thrombus toward the axillary and subclavian veins. Dialysis access, peripherally inserted central catheter placement, and thoracic outlet related venous obstruction all depend on knowing where the deep channels transition at teres major and how they relate to the humerus and surrounding soft tissues. This is also the perspective that clarifies why hematoma or fracture displacement in the midshaft humerus can threaten the adjacent brachial vessels. A tight corridor.
