Description
Posteriorly, the superficial venous network of the dorsum of the hand gathers into the dorsal venous arch and dorsal metacarpal veins, then continues proximally as a dorsal venous plexus over the distal radius and ulna. Along the lateral (radial) margin, the cephalic vein courses proximally toward the lateral arm, while the basilic vein runs on the medial (ulnar) side, both linked by smaller venules that cross the posterior forearm. At the elbow, the veins converge around the olecranon region, with communicating channels that typically connect toward the median cubital pathway on the anterior aspect. Bony landmarks, including the scapula, humerus, radius, ulna, carpals, metacarpals, and phalanges, provide orientation beneath the vessels. Posterior arm venous anatomy matters when superficial access is limited or contraindicated anteriorly, and when teaching how dorsal hand veins feed the cephalic and basilic systems. Cannulation of the dorsal venous network is common in perioperative and emergency settings, but extravasation injuries occur more often in the dorsum of the hand due to limited soft tissue and proximity to extensor tendons.
