Description
Shown within a semi-transparent male thorax, the diaphragm forms a domed musculotendinous partition spanning the inferior thoracic aperture from the xiphoid process and costal margin to the lumbar vertebrae, with its central tendon positioned superior to the liver and inferior to the heart. Anteriorly and laterally, costal fibers interdigitate with the internal intercostals along the inner surfaces of the lower ribs, while posterior crura descend toward the upper lumbar bodies. Distinct apertures are apparent: the caval opening in the central tendon on the right, the esophageal hiatus within the right crus, and the aortic hiatus posterior to the diaphragm between the crura. Understanding these hiatuses matters because they define the relationships of the inferior vena cava, esophagus, and descending aorta at a crowded transition zone where diaphragmatic motion and fascial attachments influence symptoms. Sliding and paraesophageal hiatal hernias track through the esophageal hiatus and often coexist with gastroesophageal reflux, and the illustration makes the crural sling around the esophagus easier to teach than in many surface-only views. Phrenic nerve motor supply is also clinically relevant here, since unilateral phrenic palsy produces hemidiaphragm elevation and paradoxical motion on fluoroscopy or ultrasound.
