Description
Sweeping across the inferior thoracic aperture, the diaphragm forms a domed musculotendinous partition between the thoracic cavity above and the superior abdominal viscera below. From a lateral torso perspective, you would expect to see its costal fibers arising from the internal surfaces of ribs 7 to 12 and their costal cartilages, the sternal slips near the xiphoid process, and the crura anchoring inferiorly to the anterolateral surfaces of the upper lumbar vertebral bodies, adjacent to the anterior longitudinal ligament. Posteriorly, the diaphragm relates to the thoracic and lumbar vertebral column; laterally it abuts the inner rib cage, sitting deep to the intercostal musculature and blending centrally into the tendinous center (centrum tendineum). This view matters because the diaphragm’s morphology explains both ventilation mechanics and common failure modes: phrenic nerve (C3 to C5) injury produces ipsilateral hemidiaphragm elevation and paradoxical motion, often appreciated on fluoroscopy or ultrasound, and the anatomic separation of costal and crural components helps localize dysfunction. The crura and adjacent arcuate ligaments also frame clinically relevant pathways, including the esophageal hiatus where hiatal hernia and reflux disease relate to impaired diaphragmatic pinch at the gastroesophageal junction.
