Description
Running from the posterior margin of the medial malleolus to the medial calcaneus, the flexor retinaculum (laciniate ligament) forms a dense fibrous roof over the tarsal tunnel on the posteromedial ankle. Deep to this band, the tibialis posterior, flexor digitorum longus, and flexor hallucis longus tendons course inferiorly toward the plantar foot, accompanied by the posterior tibial artery and veins and the tibial nerve. The calcaneal (Achilles) tendon lies posterior and more lateral, inserting on the calcaneal tuberosity and clearly separated from the retinacular tunnel. Bone landmarks anchoring the region include the distal tibia and fibula, talus, and calcaneus, with the subtalar and talocrural joints defining the local contours. Tarsal tunnel anatomy matters because the flexor retinaculum is the main compressive structure in tibial nerve entrapment, producing plantar paresthesia and burning pain that often worsens with prolonged standing or hindfoot valgus. Surgeons release the retinaculum during tarsal tunnel decompression and must respect the order and compartmentalization of the contents (tendons anterior, neurovascular bundle central, flexor hallucis longus posterior) to avoid iatrogenic injury. Misreading this relationship is a common teaching pitfall. Small space, big consequences.
