Description
Posterior to the medial malleolus, the flexor retinaculum (laciniate ligament) spans from the medial malleolus of the tibia to the medial surface of the calcaneus, forming the roof of the tarsal tunnel. Deep to this fibrous band, the tendons of tibialis posterior and flexor digitorum longus course inferiorly and anteriorly toward the plantar foot, while the flexor hallucis longus tendon runs more posterior and lateral in its groove on the talus and calcaneus. The posterior tibial artery and veins and the tibial nerve travel with these tendons, arranged in tight proximity along the posteromedial ankle. Clear relationships. Tight space. Compression at this site defines tarsal tunnel syndrome, where the tibial nerve becomes irritated beneath the retinaculum, often producing plantar paresthesia and medial ankle pain that worsens with prolonged standing. This posterior view also maps the anatomic corridor used when exposing or decompressing the tarsal tunnel and when assessing posterior tibial tendon dysfunction, since tendon thickening or synovitis can increase pressure under the retinaculum. Clinicians also use these landmarks to localize the posterior tibial pulse as it passes posterior to the medial malleolus and to avoid iatrogenic injury during endoscopic or open retinacular release.
