Description
Posterior skin is reflected to expose the medial ankle and heel, where the flexor retinaculum (laciniate ligament) spans from the medial malleolus of the tibia to the medial surface of the calcaneus. Deep to this fibrous band, the tendons of tibialis posterior, flexor digitorum longus, and flexor hallucis longus course inferomedially toward their plantar insertions, accompanied by the posterior tibial artery and veins and the tibial nerve within the tarsal tunnel. Laterally, the calcaneal (Achilles) tendon descends from gastrocnemius and soleus to the posterior calcaneal tuberosity, forming a clear posterior landmark relative to the retinaculum. Clinical interest centers on the tarsal tunnel itself: thickening of the flexor retinaculum, tenosynovitis of the flexor tendons, or space-occupying lesions can compress the tibial nerve and its plantar branches, producing burning plantar pain and paresthesia that often worsens with prolonged standing. Surgical decompression depends on precise orientation, releasing the retinaculum while protecting the posterior tibial neurovascular bundle and identifying the abductor hallucis fascia that can also contribute to distal entrapment. Educators can use this posterior-medial perspective in gross anatomy labs to teach the order of tarsal tunnel contents and the relationship of the retinaculum to the medial malleolus and calcaneus, and in lower-limb courses that contrast Achilles tendinopathy with medial ankle pain syndromes. It also suits podiatry, orthopedics, and radiology materials that correlate physical exam findings with ultrasound or MRI of the tibial nerve, flexor tendon sheaths, and posterior tibial vessels.
