Gluteus Medius Beneath the Skin

Description

Centered on the lateral hip, the gluteus medius is exposed beneath a translucent posterior body contour, with its broad origin spanning the external (gluteal) surface of the ilium between the anterior and posterior gluteal lines and converging inferiorly onto the lateral surface of the greater trochanter. Deep and slightly anterior to it, the gluteus minimus occupies a smaller, fan-shaped plane that also inserts on the greater trochanter, creating a stacked abductor complex lateral to the hip joint capsule. Superiorly, the posterior iliac crest and sacrum sit medial to the muscle bellies, while the proximal femora extend inferiorly, clarifying the relationship between pelvis and thigh in anatomical position. Abductor mechanism failure is a daily clinical problem. Weakness or tears of the gluteus medius and minimus, often in greater trochanteric pain syndrome, reduce pelvic stabilization in single-leg stance and produce a Trendelenburg sign with contralateral pelvic drop, a pattern you can map directly onto the muscle fiber direction and its lateral lever arm on the trochanter. Surgeons also care about this anatomy during total hip arthroplasty: splitting the gluteus medius (direct lateral approach) or detaching and repairing its insertion risks postoperative limp, and injury to the superior gluteal nerve (entering the deep surface of gluteus medius superiorly) can denervate the entire abductor unit.
