Description
Arising from the anterior inferior iliac spine and the adjacent acetabular rim, the iliofemoral ligament fans inferiorly to the intertrochanteric line of the proximal femur, forming the familiar Y shaped reinforcement across the anterior hip capsule. Medial to its lateral band, the pubofemoral ligament blends with the capsule near the pubic portion of the acetabular margin, while the femoral head sits deep within the acetabulum beneath the acetabular labrum. Superiorly, the iliac ala and lower lumbar vertebrae lead into the sacrum and coccyx, and bilaterally the coxal bones frame both hip joints in anatomical position. Thick anterior capsular ligaments matter because they explain why the resting hip tends toward extension without constant muscular effort and why hyperextension stresses the joint even in a normal gait cycle. The iliofemoral ligament is the primary checkrein to extension and external rotation, so laxity after anterior dislocation, capsulotomy, or chronic instability changes both exam findings and operative planning. A key landmark. Its relationship to the femoral neck and intertrochanteric line also orients the surgeon during anterior and anterolateral approaches, where capsular management can influence postoperative stability. Orthopedic and sports medicine educators will find this view suited for teaching hip stability, capsular anatomy, and the distinction between iliofemoral and pubofemoral reinforcements in gross anatomy and musculoskeletal courses.
