Description
Deep to the skin of the anterior neck and upper thorax, the inferior oblique fibers of longus colli run superolaterally from the anterolateral bodies and transverse processes of upper thoracic vertebrae (classically T1 to T3) to the anterior tubercles of midcervical transverse processes (often C5 to C6). Medial to these fibers lie the cervical vertebral bodies and intervertebral discs, while the scalenes sit more laterally, spanning from cervical transverse processes toward the first ribs. Superiorly the mandible frames the upper margin of the field, and inferiorly the thoracic cage and clavicle establish the transition into the root of the neck; the scapula appears posterolateral as part of the shoulder girdle. Surgeons and trainees rely on longus colli as a prevertebral landmark during anterior cervical discectomy and fusion and other anterior approaches to the cervical spine, where the muscle is elevated off the vertebral bodies to expose the disc space and uncovertebral region. Its lateral border is where you think about the cervical sympathetic trunk and stellate ganglion, and excessive lateral dissection can contribute to postoperative Horner syndrome. Retropharyngeal edema or prevertebral hematoma also tracks along this plane, so understanding the muscle’s depth and fascial relationships helps interpret anterior neck swelling and airway risk after trauma or surgery.
