Description
Centered deep to the infrahyoid strap muscles, the inferior oblique portion of the longus colli is rendered on the anterior aspect of the cervical spine, with its short, diagonally oriented fibers running superolaterally from upper thoracic vertebral bodies toward the lower cervical transverse processes. Superiorly, the hyoid-associated region and laryngeal framework sit anterior to the prevertebral compartment, while the thyroid and cricoid cartilages mark the midline airway column in front of the vertebrae. Clavicles bracket the root of the neck laterally, and the sternum and upper ribs anchor the thoracic inlet inferiorly. Bone remains off-white, muscle red, so the prevertebral muscle layer reads clearly even under an intact skin envelope. For anterior cervical approaches, this anatomy matters because longus colli defines the medial border of the surgical corridor and provides a safe plane for subperiosteal dissection to expose the vertebral bodies and disc spaces without drifting into the carotid sheath laterally. Misidentifying the muscle or stripping it unevenly can increase bleeding and postoperative dysphagia, and it also risks traction on the sympathetic chain that lies along its lateral margin, a known mechanism for Horner syndrome after anterior cervical discectomy and fusion. Small muscle, big consequences. The inferior oblique fibers also help interpret prevertebral edema in acute whiplash and localize calcific tendinitis of longus colli, which classically causes acute neck pain and odynophagia with retropharyngeal effusion on CT. Use this anterior male neck plate for gross anatomy teaching on the prevertebral layer, for spine surgery atlases illustrating the ACDF exposure, or for radiology and emergency medicine education linking prevertebral soft tissue swelling to longus colli pathology. It also fits well in otolaryngology texts that contrast the visceral compartment (larynx, pharynx) with the prevertebral compartment at the cervical level.
