Running along the anterior surface of the cervical spine, the longus colli is exposed deep to the visceral column of the neck, with emphasis on its inferior oblique part (obliquus inferior). Slender muscle slips arise from the anterolateral vertebral bodies of the upper thoracic spine (classically T1 to T3) and course superolaterally to attach by short tendinous segments onto the anterior tubercles of the transverse processes of mid cervical vertebrae (commonly C5 to C6). Superiorly, the field includes the mandible and hyoid bone, while the thyroid cartilage sits inferior to the hyoid in the anterior midline; inferiorly, the upper ribs and sternum frame the cervicothoracic junction. Deep anatomy. Clear attachments. For anterior cervical discectomy and fusion and other anterior approaches to the cervical spine, the longus colli forms the medial border of the operative corridor and is often elevated off the vertebral bodies to place retractors. That maneuver matters because the cervical sympathetic trunk runs just lateral to longus colli on the prevertebral fascia, so aggressive lateral dissection can contribute to postoperative Horner syndrome. The inferior oblique fibers also help explain patterns of prevertebral swelling or hematoma that can track along the longus colli plane and narrow the retropharyngeal space, a practical teaching point when correlating postoperative airway risk.
