Description
Deep to the sternocleidomastoid and infrahyoid strap region, the inferior oblique part of the longus colli is shown coursing superolaterally across the anterior lower cervical spine. Its fibers arise from the anterior surfaces of the upper thoracic vertebral bodies (classically T1 to T3) and ascend to the anterior tubercles of the transverse processes of C5 and C6, placing it lateral to the midline longus colli vertical fibers and anterior to the vertebral bodies and intervertebral discs. Superiorly, the hyoid and laryngeal framework sit anterior to the prevertebral compartment, while the clavicle and first ribs define the inferior thoracic inlet boundary. A deep muscle. This segment of longus colli matters because it forms the prevertebral muscular bed encountered during an anterior cervical approach, where retraction and subperiosteal dissection occur along the longus colli margins to expose the disc space. The cervical sympathetic trunk runs on or just lateral to the longus colli fascia, so aggressive lateral dissection can produce postoperative Horner syndrome. Radiologists and emergency clinicians also recognize longus colli calcific tendinitis at the C1 to C3 region, but the associated prevertebral soft tissue swelling tracks inferiorly, so understanding the prevertebral planes in the lower cervical region helps interpret retropharyngeal edema and odynophagia on CT.
