Description
Posteriorly, the suboccipital region is opened beneath the skin to isolate the inferior obliquus capitis (obliquus capitis inferior) coursing superolaterally from the spinous process of the axis (C2) to the transverse process of the atlas (C1). Just superior and medial to it, the rectus capitis posterior major and minor sit against the inferior nuchal line of the occipital bone, framing the atlanto occipital and atlanto axial complexes. The atlas and axis are rendered with their characteristic ring and dens relationships, while the intervertebral discs and posterior ligamentous elements align inferiorly along the cervical column. Small space. Big consequences. Because the inferior obliquus capitis is the only suboccipital muscle that does not attach to the skull, its line of pull is a clean teaching example for C1 on C2 rotation and for how head turning is coupled to upper cervical joint mechanics. Clinically, this area matters in cervicogenic headache and post whiplash pain patterns, where hypertonicity or myofascial trigger points in the suboccipital muscles can refer pain to the occiput and behind the eye, and where the suboccipital nerve (dorsal ramus of C1) may be irritated as it traverses the suboccipital triangle adjacent to the vertebral artery.
