Description
Posterior scalp and upper cervical layers are rendered with the skin reflected to expose the occipitalis over the superior nuchal line and the superficial nuchal musculature. Deep to these planes, the inferior obliquus capitis is highlighted near the craniovertebral junction, running superolaterally from the spinous process of C2 (axis) to the transverse process of C1 (atlas). Its fibers sit inferior to rectus capitis posterior major and lateral to rectus capitis posterior minor, forming the inferolateral border of the suboccipital triangle against the posterior arch of the atlas. Orientation is unmistakable. That relationship matters because the suboccipital triangle is a common site of neurovascular crowding, where the vertebral artery (V3 segment) courses in a groove on the atlas and the suboccipital nerve (dorsal ramus of C1) emerges between atlas and occiput. Hypertonicity or spasm of the inferior obliquus capitis can contribute to cervicogenic headache patterns, and its proximity to the greater occipital nerve as it pierces semispinalis capitis helps explain why posterior neck pain can radiate to the occiput and scalp. For posterior approaches to the atlantoaxial complex, the muscle’s attachment points on C1 and C2 are reliable landmarks when defining the safe dissection corridor. Faculty can drop this plate directly into gross anatomy and neuroanatomy teaching on the suboccipital region, including fiber direction, layered dissection logic, and boundaries of the suboccipital triangle.
