Posteriorly, the inferior obliquus capitis (obliquus capitis inferior) stands out as a short, thick suboccipital muscle running obliquely from the spinous process of the axis (C2) superolaterally to the transverse process of the atlas (C1). Lateral to the midline nuchal ligament and deep to the semispinalis capitis, its fibers sit inferior to the rectus capitis posterior major and adjacent to the superior obliquus capitis, completing the functional cluster that frames the suboccipital region. The occipital bone and posterior arch of C1 form the superior and deep backdrop, while more superficial layers such as splenius capitis and upper trapezius may be reflected or partially visible, depending on the dissection depth.
