Description
Positioned deep in the posterior neck, the obliquus capitis inferior runs from the spinous process of the axis (C2) superolaterally to the transverse process of the atlas (C1). The paired muscles sit inferior to the posterior occiput and medial to the superior margin of the scapulae in this posterior view, with the cervical vertebrae forming the midline column beneath the external occipital region. Surrounding osseous landmarks include the occipital bone, posterior arch of C1, and the prominent C2 spinous process, with the upper thoracic vertebrae descending inferiorly. Rotation lives here. Contraction of obliquus capitis inferior drives ipsilateral atlantoaxial rotation and helps stabilize C1 on C2 during fine head movements, making it a teaching staple for separating atlanto-occipital nodding from atlantoaxial turning. Clinically, this muscle forms the inferior boundary of the suboccipital triangle and sits adjacent to the V3 segment of the vertebral artery and the suboccipital nerve (dorsal ramus of C1), relationships that matter in posterior upper cervical approaches and in discussions of cervicogenic headache mechanisms linked to suboccipital tension and C1-C2 dysfunction.
