Description
Arising from the spinous process of the axis (C2), the obliquus capitis inferior runs superolaterally to insert on the transverse process of the atlas (C1), forming a short, thick band in the deep posterior neck of an adult male. Posterior to the upper cervical vertebrae, it sits inferior to the rectus capitis posterior major and just deep to semispinalis capitis, with its fibers oriented obliquely across the C1 to C2 interval. Laterally, the muscle approaches the posterior arch of the atlas; medially, it frames the interspinous region of C2. Small, on purpose. That C1 to C2 relationship is why this muscle matters: it is the primary driver of atlantoaxial rotation, the motion you test when you ask a patient to turn the head left and right. Its borders define part of the suboccipital triangle, a key dissection landmark for locating the vertebral artery as it grooves the posterior arch of C1 before entering the foramen magnum, and for identifying the suboccipital nerve (dorsal ramus of C1) coursing in the same confined space. Surgical exposure in this region, whether for C1 lateral mass instrumentation or posterior decompression, demands respect for that anatomy. Ideal for teaching deep posterior cervical musculature in gross anatomy, neuroanatomy, and spine surgery courses, this view also supports atlas-axis biomechanics figures in orthopedic and neurosurgical texts and patient-facing explanations of cervicogenic headache sources.
