Description
Deep to the posterior cervical fascia, the suboccipital musculature is exposed against the occipital bone and upper cervical vertebrae, with the inferior obliquus capitis coursing obliquely from the spinous process of C2 (axis) to the transverse process of C1 (atlas). Medial to it, rectus capitis posterior major ascends from the C2 spinous process to the inferior nuchal line, while the surrounding soft tissue planes suggest the overlying splenius capitis and semispinalis capitis have been reflected. Bony landmarks of the external occipital region and the posterior arch region of C1 frame the muscle attachments. Small and deep. That oblique C2 to C1 trajectory matters because it is the primary muscular driver of ipsilateral atlantoaxial rotation, and it forms the inferolateral boundary of the suboccipital triangle where the vertebral artery and suboccipital nerve (dorsal ramus of C1) run in close proximity. Tightness, strain, or postural overload in the inferior obliquus capitis is a common target in discussions of cervicogenic headache, especially when paired with irritation of the greater occipital nerve as it pierces semispinalis capitis more inferiorly.
