Description
Running obliquely across the upper cervical spine, the inferior obliquus capitis extends from the spinous process of the axis (C2) to the transverse process of the atlas (C1) on the posterolateral aspect of the neck. A lateral perspective places the muscle deep to the more superficial posterior cervical layers, with the occipital bone and the posterior arch of C1 lying superior, and the C2 lamina and spinous process forming its inferomedial attachment. Anteriorly, the mandible and upper airway framework sit well forward of this suboccipital region, while the clavicle, scapula, and upper ribs anchor the lower frame of the composition. C1 to C2 rotation depends heavily on this short muscle and its suboccipital partners, and the lateral view clarifies how a relatively small belly can exert a strong rotational moment on the atlas while remaining tucked beneath larger extensors such as semispinalis capitis and splenius capitis. Small muscle, big symptoms. Hypertonicity or strain here is a common contributor to cervicogenic headache and occipital pain patterns, and its location adjacent to the C2 dorsal ramus and the greater occipital nerve makes it a frequent suspect in occipital neuralgia after whiplash; it also borders the suboccipital triangle, where the vertebral artery and suboccipital nerve (C1 dorsal ramus) are classically discussed in surgical anatomy and regional blocks.
