Description
Seen from the anterior aspect of an adult male pelvis, the obturator externus forms a broad, triangular muscular sheet spanning the obturator foramen on the external surface of the obturator membrane, bordered by the pubis and ischium. Its fibers converge posterolaterally toward the hip joint, passing inferior to the acetabulum and femoral head to reach the trochanteric fossa of the proximal femur. Surrounding bony landmarks include the ilium superiorly, the sacrum and coccyx in the midline posteriorly, and the paired acetabula laterally where the femoral heads articulate. Hip rotator musculature clusters around the acetabular rim. Clear orientation matters. An anterior rendering of obturator externus is less commonly taught than posterior deep gluteal views, yet it clarifies the muscle’s relationship to the obturator foramen and the anterior pelvic boundary, which helps explain its line of pull as an external rotator and secondary adductor of the thigh. This perspective also supports surgical anatomy: the obturator externus tendon lies close to the inferomedial hip capsule, a neighborhood relevant during anterior or anterolateral approaches to total hip arthroplasty and when evaluating obturator externus strain or tendinopathy as a cause of deep groin pain. The muscle’s proximity to the obturator canal and neurovascular bundle also makes it a useful reference when teaching obturator nerve entrapment patterns. Ideal for gross anatomy teaching on the pelvis and hip, sports medicine content on hip rotator injuries, and orthopedic or radiology publications discussing groin pain differentials and periarticular hip anatomy.
