Description

Centered on the bilateral kidneys, the renal veins are traced from the intrarenal venous tributaries draining cortex and medulla toward the renal hila, where they converge into right and left renal veins. Medial to each kidney, these veins course horizontally to the inferior vena cava, while the renal arteries arise from the adjacent abdominal aorta and pass laterally into the hilum in close anteroposterior relationship with the renal pelvis and proximal ureter. Beige ureters descend inferiorly from the renal pelvis, and perirenal adipose tissue surrounds the renal parenchyma, forming a recognizable cushion around the organ contours. Green neural elements consistent with the renal plexus are shown accompanying the renal vessels near the hilum. Clear vascular anatomy. Renal venous anatomy matters when planning interventions at the renal hilum and retroperitoneum, where small changes in vein length and position affect exposure and safe ligation. The short right renal vein entering the inferior vena cava is a known constraint in right donor nephrectomy and renal transplantation, while the longer left renal vein (often receiving the left gonadal and suprarenal veins) is favored for graft harvest and is the vessel implicated in nutcracker syndrome when compressed between the aorta and superior mesenteric artery. Venous outflow is also central to renal cell carcinoma staging, since tumor thrombus can extend from a renal vein into the inferior vena cava.
