Description
Sagittal section through an adult male kidney reveals the intrarenal venous tree in blue converging from the cortex and medulla toward the renal sinus and hilum. Interlobular veins course radially through the cortex, draining into arcuate veins that arch along the corticomedullary junction before feeding interlobar veins that run between adjacent renal pyramids toward the renal vein. Red arterial branches accompany the veins in parallel, while yellow structures within the sinus are consistent with the renal pelvis and calyceal system abutting the vessels. Venous anatomy inside the kidney matters because it maps directly onto the segmental organization that surgeons and interventionalists must respect when approaching the hilum. During partial nephrectomy or hilar dissection, injury to an interlobar vein or an early confluence into the renal vein can produce brisk, obscuring hemorrhage, and the close relationship of veins to the collecting system helps explain postoperative hematuria or urinary leak when planes are violated. Venous congestion also underlies imaging findings in renal vein thrombosis, where impaired outflow propagates backward into the intrarenal veins and can mimic or accompany parenchymal edema.
