Description
Centered within a vertebral canal segment, the spinal cord is rendered in transverse section with butterfly-shaped gray matter surrounded by white matter tracts. Posterior (dorsal) horns project toward the dorsal aspect, while anterior (ventral) horns broaden toward the ventral surface, framing the central canal at the midline. Laterally, paired rootlets converge as dorsal and ventral roots to form a mixed spinal nerve near the intervertebral foramen. A blue circumferential zone represents cerebrospinal fluid within the subarachnoid space, separating the cord from the surrounding meninges and osseous vertebra. CSF and meninges matter most where mechanics and pathology intersect. This view clarifies why a dorsal approach for lumbar puncture targets the subarachnoid space, and why the needle must pass sequentially through ligamentum flavum, epidural space, dura mater, and arachnoid mater before CSF return, while avoiding the spinal cord proper by sampling below the conus medullaris. It also sets up the logic of cord compression: vertebral canal stenosis, epidural hematoma, or intradural extramedullary tumors distort the CSF cushion first, then deform white matter tracts, often producing early dorsal column signs such as loss of vibration and proprioception.
