Description
Shown in transverse section, the spinal cord sits centrally within the vertebral canal, with butterfly-shaped gray matter (anterior and posterior horns joined by the gray commissure) surrounded by peripheral white matter funiculi. A small central canal occupies the midline and aligns with the cord’s longitudinal axis, while the pia mater closely invests the cord surface. Cerebrospinal fluid fills the subarachnoid space between pia and arachnoid, and a tougher dural sleeve forms the outer meningeal boundary, separated from the vertebral wall by the epidural space within the bony vertebra. This cutaway is the teaching workhorse for linking cord microanatomy to neurologic deficits: anterior horn involvement explains lower motor neuron weakness, posterior horn and dorsal columns map to sensory loss, and lateral white matter highlights the corticospinal tracts targeted in many “where is the lesion” questions. The CSF compartment matters clinically, not as thermal buffering, but because lumbar puncture samples CSF from the subarachnoid space and pathologies like spinal epidural hematoma, meningitis, and subarachnoid hemorrhage are defined by their relationship to dura, arachnoid, and pia. Spatial boundaries are the point. They guide needle placement, imaging interpretation, and surgical decompression planning.
