Seen in lateral (sagittal) profile, the telencephalon is opened to expose the deep gray nuclei that form the basal ganglia, with the putamen lying lateral to the globus pallidus and both positioned inferior to the body of the lateral ventricle and deep to the insular cortex. Medial to this striatal complex, the thalamus is highlighted in blue as it sits in the diencephalon, superior to the midbrain and bordering the third ventricle. Inferiorly, the brown brainstem continues caudally toward the spinal cord, while the cerebellum appears posteroinferior to the occipital lobe, separated from the cerebrum by the tentorial plane. This lateral perspective matters because it clarifies how the pallidum and putamen relate to the thalamus and brainstem along the major motor loops. Those circuits are the anatomic substrate for Parkinson disease (pallidal and subthalamic network dysfunction), Huntington disease (striatal degeneration), and hemiballismus (subthalamic nucleus lesions, often taught in the same spatial neighborhood even if not fully exposed in a strict sagittal cut). Orientation is everything. The view also supports discussion of stereotactic trajectories for deep brain stimulation targets (GPi and STN) by anchoring them to the thalamus and midbrain in a side-on layout that mirrors common planning screenshots.
