Presented in lateral profile, the skull and cervical spine frame the extracranial course of the facial nerve (CN VII) from the stylomastoid foramen, immediately posterior to the mandibular ramus and inferior to the external acoustic meatus. From the parotid region, the temporofacial and cervicofacial divisions form the parotid plexus and give rise to temporal, zygomatic, buccal, marginal mandibular, and cervical branches that fan anteriorly across the cheek toward the orbicularis oculi and oris. A ganglion is suggested at the proximal segment of the nerve’s pathway, with neighboring cranial nerve trajectories implied around the temporal bone and upper neck. Color coding separates neural structures from adjacent tissues. Clear and legible. This lateral perspective matters because it tracks the surface-projection of CN VII branches across surgical planes that are routinely violated in parotidectomy, facelift dissection (SMAS plane), and mandibular angle approaches. The marginal mandibular branch, typically coursing near the inferior border of the mandible before turning superiorly toward depressor anguli oris, is a common site of iatrogenic injury and postoperative asymmetry. Orientation to the stylomastoid foramen, parotid plexus, and mandibular ramus also helps explain patterns of facial palsy, including Bell palsy and lesions proximal to the geniculate ganglion that may combine facial weakness with altered lacrimation or taste.
