Description
Arising from the dorsal ramus of C2, the greater occipital nerve is traced in lateral profile as it courses superiorly from the upper cervical spine toward the posterior scalp, lying deep to the occipital region before becoming superficial near the superior nuchal line. Along its ascent it relates closely to the posterior arch region of the atlas and axis, then passes between the inferior oblique and semispinalis capitis, continuing toward the occipital bone just posterior to the mastoid region. A dense field of superficial sensory branches spreads over the parietal and occipital scalp. Skeletal landmarks of the cranium, mandible, dentition, and the cervical vertebrae anchor the nerve’s pathway in clear anatomic space. Lateral orientation matters here because entrapment and irritation most often occur where the nerve pierces semispinalis capitis and the trapezius aponeurosis near the nuchal line, a common generator of occipital neuralgia with pain radiating to the vertex and sometimes toward the retroauricular area. Relationship to the occipital artery and to adjacent lesser occipital and auricular branches helps distinguish dermatomal patterns that can mimic cervical facet referral or migraine.
