Description
From a lateral perspective, the pedal skeleton is arranged from the posterior calcaneus and talus through the midfoot tarsals, into the metatarsals and on to the phalanges of the lesser toes, with the phalangeal elements highlighted in blue. Attention centers on the middle phalanx, where a narrow diaphysis (shaft) lies between an expanded proximal base and a broader distal head, forming the bony core between the proximal and distal interphalangeal joints. Distally, each middle phalanx sits distal to its proximal phalanx and proximal to its distal phalanx, aligned along the longitudinal axis of the toe. Joint surfaces are indicated by lighter blue cartilage bands. A true lateral view clarifies how the middle phalanx contributes to toe “rocker” mechanics during terminal stance, and it makes the sagittal plane contour of the interphalangeal joints easy to interpret. This is the angle you want when teaching or discussing hammertoe and mallet toe deformities, where chronic flexion at the proximal interphalangeal joint concentrates pressure dorsally over the middle phalanx and plantarward at the distal tip. It also maps cleanly onto the standard lateral forefoot radiograph used to assess phalangeal fractures and post-reduction alignment. Small bone, common problem.
