Description
Seen in lateral profile, the distal phalanx of a toe sits at the far distal end of the digital ray, with its base articulating proximally at the distal interphalangeal joint (or the interphalangeal joint in the hallux) and its apex directed distally toward the nail bed. The shaft tapers anteriorly and distally, while the plantar surface broadens into the tuft that supports the fibrofatty pulp. Dorsally, the ungual process underlies the nail plate, and the distal phalanx lies anterior to the tarsal and metatarsal complex that recedes proximally toward the calcaneus and talus in the same lateral projection. Lateral views of the distal phalanx matter because this bone is the osseous endpoint for toe-tip loading and a frequent site of crush injury, subungual hematoma, and open tuft fractures. Small avulsion fragments at the dorsal base correspond to extensor tendon insertion patterns, while plantar pain and swelling can reflect infection tracking to the distal phalanx in diabetic foot disease or an ingrown toenail complicated by osteomyelitis. In surgical planning, the contour of the tuft and the relationship to the distal interphalangeal joint guide debridement margins and distal digital amputations.
