Description
Inferiorly oriented, this plate isolates the proximal phalanges of the toes against the distal metatarsals and the anterior tarsus. Each proximal phalanx presents a broadened base proximally for the metatarsophalangeal joint, a shaft tapering distally, and a rounded head that articulates with the middle phalanx in digits II to V, while the hallux continues to the distal phalanx without an intervening middle phalanx. The plantar (inferior) aspects of the phalangeal bases lie distal to the metatarsal heads, with the heads of the proximal phalanges sitting proximal to the proximal interphalangeal joints and aligned in a gentle medial to lateral arc. Joint margins are cleanly defined. Bone only, no distractions. Plantar views of the proximal phalanx matter because they align directly with the functional surface that meets the ground through the plantar plate and flexor tendon apparatus at the metatarsophalangeal level. Hammertoe and claw toe deformities often start with sagittal plane imbalance at the metatarsophalangeal joint, then drive hyperextension proximally and flexion at the interphalangeal joints, and this perspective helps clarify where the proximal phalangeal base and head constrain motion. It also supports preoperative planning for proximal interphalangeal arthrodesis and phalangeal osteotomy, where understanding the contour of the proximal phalanx guides fixation trajectory and avoids joint penetration.
