Seen from the dorsal (superior) aspect, the proximal phalanges of digits II to V sit distal to their respective metatarsal heads, with broad bases forming the metatarsophalangeal joints and tapered shafts running distally toward the middle phalanges. Medially, the hallux is identifiable by its single proximal phalanx articulating with the first metatarsal and continuing to a distal phalanx without an intermediate segment. Interphalangeal joint lines and metatarsophalangeal joint spaces are indicated by thin cartilage bands, helping separate the proximal phalanges from adjacent phalanges and the metatarsals. Orientation is clear: metatarsals and tarsals lie proximal and posterior relative to the toes, while the distal phalanges mark the anterior limit of the forefoot. A superior view of the proximal phalanx and metatarsophalangeal region is where alignment problems declare themselves. Hallux valgus and hallux rigidus both center on the first metatarsophalangeal joint, and the contour of the proximal phalanx base and its relationship to the first metatarsal head underpins radiographic assessment and operative planning (chevron and Akin osteotomies are common examples). Lesser-toe deformities, including hammer toe and claw toe, also hinge on metatarsophalangeal subluxation and altered joint congruity. Small bones, big consequences.
