Description
Running vertically along the anterior cervical region, the paired sternohyoid muscles form the most superficial infrahyoid strap layer, extending from the posterior surface of the manubrium and medial clavicle inferiorly to the body of the hyoid bone superiorly. In lateral perspective, their narrow bellies track anterior and slightly medial to the laryngeal framework, sitting deep to the platysma and superficial cervical fascia, while the sternocleidomastoid lies more lateral and oblique from mastoid process to sternum and clavicle. Superiorly, the mandible and facial musculature (including orbicularis oculi and masseter) provide craniofacial context, and posteriorly the upper fibers of trapezius frame the posterior triangle behind the sternocleidomastoid. The external ear anchors the side profile. For surgeons and proceduralists, this arrangement matters because the strap muscles act as the first muscular plane encountered when you open the midline of the anterior neck, and their separation or retraction exposes the thyroid gland, trachea, and larynx during thyroidectomy, tracheostomy, and laryngotracheal approaches. Midline splitting between the sternohyoids reduces bleeding compared with a more lateral dissection near the carotid sheath, while preserving their innervation from the ansa cervicalis helps limit postoperative dysphonia and swallowing complaints related to impaired hyoid depression.
