Description
Running along the anterior neck in lateral profile, the sternothyroid muscle forms a deep strap that ascends from the posterior surface of the manubrium and adjacent first costal cartilage to the oblique line of the thyroid cartilage. Superficial to it, the sternohyoid parallels the same vertical axis toward the hyoid bone, while the sternocleidomastoid crosses obliquely from the mastoid process to the sternum and clavicle, partially roofing the infrahyoid compartment. Posteriorly, the cervical vertebrae align beneath the cranial base, and inferiorly the clavicle, scapula, and upper ribs anchor the shoulder girdle and thoracic inlet. Bony landmarks stay readable. A lateral view is where the layered architecture of the strap muscles makes sense, since you can appreciate sternothyroid sitting deep to sternohyoid and inserting directly on the laryngeal framework rather than the hyoid. That relationship matters in thyroid and parathyroid surgery: midline separation and lateral retraction of the infrahyoid muscles exposes the thyroid gland, and awareness of sternothyroid attachments helps explain postoperative dysphonia or tethering when the larynx is manipulated. Clinically, it also supports teaching why superior laryngeal excursion during swallowing depends on suprahyoid elevation and coordinated infrahyoid relaxation.
