Description
Running in the anterior midline of the neck, the paired sternohyoid muscles appear as flat, ribbon-like infrahyoid straps lying immediately deep to the skin and superficial cervical fascia, medial to the sternocleidomastoid on each side. Each belly ascends superiorly from the posterior aspect of the manubrium and adjacent medial clavicle to approach the hyoid region, maintaining a close relationship to the sternum inferiorly and the laryngeal framework deep to it. Lateral bony landmarks include the clavicles and upper ribs at the thoracic inlet, while superiorly the mandible, maxilla, nasal cavity, and dentition establish a craniofacial frame for the cervical anatomy. The anterior neck is presented symmetrically, emphasizing left-right strap muscle alignment. Clinically, sternohyoid is a key surface landmark for orienting the infrahyoid layer during tracheostomy and thyroid surgery, where midline dissection between strap muscles reduces bleeding and preserves the ansa cervicalis branches that enter the muscle on its deep surface. Its position anterior to the larynx and upper trachea explains why strap muscle edema, postoperative scarring, or hematoma can contribute to dysphagia and restricted laryngeal excursion after neck procedures.
