Description
Seen from the lateral aspect of an adult male thigh, the lateral circumflex femoral artery arises from the profunda femoris (deep artery of the thigh) near the femoral triangle and courses laterally, deep to sartorius and rectus femoris, toward the proximal femur. Near the intertrochanteric region it divides into the ascending branch running superiorly toward the greater trochanter, the transverse branch passing posteriorly around the femur, and the descending branch tracking inferiorly along the lateral thigh. Red arterial channels and blue venous counterparts trace alongside each other, with the deep venous return accompanying the arterial branches while more superficial veins remain closer to the fascia lata. Understanding this branching pattern matters because the descending branch is the workhorse pedicle for the anterolateral thigh (ALT) flap and a frequent target in perforator-based reconstruction. Surgeons also rely on the ascending and transverse branches for the trochanteric and cruciate anastomoses, collateral pathways that can preserve perfusion when the superficial femoral artery is compromised. Bleeding here is unforgiving.
