Description

Seen from the lateral aspect of the male ankle and foot, the fibula terminates inferiorly as the lateral malleolus, lateral to the distal tibia and talus at the talocrural (ankle) joint. The lateral malleolar artery courses posterior to the lateral malleolus, wrapping around its bony contour before contributing to the lateral malleolar anastomosis over the lateral hindfoot. Distally, the tarsal bones, metatarsals, and phalanges provide the osseous framework, with accompanying arteries in red and superficial and deep venous channels in blue tracking the same compartments. A lateral perspective is where the relationship between the lateral malleolus and perimalleolar vessels becomes clear. This is the corridor encountered in lateral ankle approaches and during fibular fracture work, where swelling or hardware placement can compromise small arterial branches and contribute to delayed wound healing over the lateral malleolus. Palpation and Doppler assessment around the fibular tip also rely on understanding how the lateral malleolar artery swings posteriorly before turning anteriorly along the ankle.
