Description
Seen from the plantar aspect, the lateral plantar artery courses obliquely from the posterolateral heel, emerging as the larger terminal branch of the posterior tibial artery deep to the flexor retinaculum and abductor hallucis. It then sweeps anteriorly and medially across the sole between the flexor digitorum brevis (superficial) and quadratus plantae (deep), approaching the bases of the metatarsals where it continues toward the deep plantar arch. Bony landmarks such as the calcaneus, talus, cuboid, cuneiforms, metatarsals, and phalanges provide clear positional reference, with accompanying plantar veins tracking parallel to the arterial path. A short segment of the distal tibia and fibula frames the proximal inflow. Clinically, this artery matters because it supplies much of the lateral plantar compartment and forms the main contributor to the plantar arterial arch, a key collateral route between posterior tibial and dorsalis pedis circulation. Its deep course between muscular layers and proximity to the lateral and medial plantar nerves makes it a structure to respect during plantar fasciotomy, tarsal tunnel release, and deep dissection for midfoot procedures. Injury or occlusion here can compromise forefoot perfusion, and its branches are relevant when planning flaps or interpreting ischemic patterns in diabetic foot disease. Deep and unforgiving.
