Description
Plantar dissection orientation highlights the lateral plantar veins coursing deep within the sole, running anteriorly from the lateral calcaneal region along the lateral longitudinal arch toward the bases of the metatarsals. These venae comitantes track alongside the lateral plantar artery and lie inferior to the tarsal and metatarsal bones, with the calcaneus and cuboid forming the posterior and lateral bony framework and the metatarsals and phalanges extending distally. Proximally, the venous channels converge toward the hindfoot to continue into the posterior tibial venous system, with the tibia and fibula included as proximal landmarks for orientation. Deep to the plantar fascia, the veins sit among the intrinsic plantar muscles and adjacent neurovascular structures. Small but clinically unforgiving. Understanding the lateral plantar venous pathway helps clarify why plantar venous drainage can become symptomatic under repetitive load, immobilization, or after hindfoot surgery, when calf pump mechanics and plantar muscular contraction are reduced. For clinicians interpreting duplex ultrasound or MR venography, the relationship of these deep sole veins to the lateral plantar artery and tibial nerve branches supports accurate localization of a suspected plantar venous thrombosis or postoperative hematoma in the lateral compartment of the sole. This plantar perspective also reinforces the concept that much of the foot’s venous return is deep and paired, not limited to the superficial dorsal venous arch.
