Description
Arising from the posterior tibial artery just distal to the medial malleolus, the lateral plantar artery courses obliquely and laterally across the plantar surface, deep to the plantar aponeurosis and superficial to the deeper interosseous plane. Along its path it passes anterior to the calcaneal tuberosity, then runs toward the bases of the metatarsals, where it turns medially to contribute to the deep plantar arterial arch. Adjacent soft tissues typically include the flexor digitorum brevis and abductor digiti minimi in the superficial layer, with slips of quadratus plantae and the flexor hallucis longus tendon deeper and more medial. This is a dense corridor. For surgical anatomy, a plantar perspective clarifies why the lateral plantar artery is at risk during deep plantar dissections and medial or lateral approaches to midfoot pathology, including debridement for plantar abscess or exposure during reconstruction of the tarsometatarsal region. The vessel’s relationship to the plantar musculature and the expected site of the arterial arch also frames common patterns of ischemia in peripheral arterial disease and informs flap planning for plantar soft-tissue coverage, where preserving or deliberately ligating branches changes distal perfusion. Spatial orientation matters.
