Description
Arising as a terminal branch of the posterior tibial artery beneath the flexor retinaculum, the medial plantar artery courses anteromedially across the plantar aspect of the foot, deep to abductor hallucis and along the medial side of flexor digitorum brevis toward the hallux. Plantar to the tarsals and metatarsals, it runs in parallel with the medial plantar nerve and remains closely related to the long flexor tendons (flexor hallucis longus and flexor digitorum longus) as they sweep distal to the medial malleolus. Nearby landmarks include the calcaneal tuberosity posteriorly, the talus and navicular medially, and the first metatarsal and metatarsophalangeal region distally. Clear layering. Bone, tendon, and vessel planes remain distinct. Surgical and diagnostic work around the medial ankle and medial plantar midfoot depends on respecting this neurovascular bundle, since the artery can be injured during tarsal tunnel release, plantar fascia procedures, or medial approach fixation near the navicular and talus. The plantar perspective also clarifies why medial plantar artery compromise tends to present as exertional plantar arch pain or delayed wound healing along medial forefoot incisions, given its supply to the medial plantar musculature and contributions toward the superficial and deep plantar arterial networks. This is a common blind spot when learners only study dorsal foot perfusion. Podiatry and orthopedic teaching files can pair this plate with tendon anatomy (FDL, FHL) and the flexor retinaculum to explain safe corridors for medial hindfoot and midfoot approaches, as well as flap planning and avoidance of iatrogenic vascular injury.
