Description

Arising from the posterior tibial artery just distal to the medial malleolus, the medial plantar artery tracks anteriorly along the medial plantar aspect of the foot, deep to the abductor hallucis and in company with the medial plantar nerve. From a medial perspective, its relationship to the calcaneal tuberosity posteriorly and the first metatarsal and hallux region distally reads clearly, with accompanying veins (venae comitantes) flanking the artery. Proximal leg vessels and nerves are carried into the field to orient the plantar course relative to the tarsal tunnel and the medial border of the sole. Medial plantar vascular anatomy matters any time you work near the tarsal tunnel or plan incisions along the instep. The artery and medial plantar nerve can be compromised in tarsal tunnel syndrome, and their close pairing helps explain mixed ischemic and neuropathic symptoms in medial plantar heel and forefoot pain.
