Description
Arising from the anterior tibial artery near the ankle, the lateral malleolar artery curves inferiorly and laterally across the anterior aspect of the distal fibula toward the lateral malleolus, where it runs close to the lateral joint capsule. Distal branches track over the tarsus toward the dorsum of the foot, in continuity with the dorsal arterial network supplying the metatarsals and phalanges. Veins accompany the arterial pattern superficially and deep, converging proximally toward the anterior tibial venous system. Bone landmarks anchor the course, with the fibula lateral to the tibia and the talus and calcaneus forming the hindfoot immediately inferior. Course matters here. The lateral malleolar artery participates in periarticular anastomoses around the ankle, commonly linking with the perforating branch of the fibular (peroneal) artery and branches of the dorsalis pedis, which helps explain why distal perfusion can persist when one inflow channel is compromised. Its proximity to the anterolateral ankle capsule places it in the operative field during ankle arthroscopy portal placement and open approaches to the distal fibula, and it can contribute to troublesome bleeding or postoperative hematoma if not anticipated. A small vessel, but a real landmark. Use this plate in lower limb anatomy teaching to correlate the bony architecture of the tarsus and metatarsus with the dorsal arterial tree, or in surgical education materials covering anterolateral ankle exposure, lateral malleolus fixation, and vascular considerations in trauma.
