Description
Running along the anterolateral aspect of the distal leg, the lateral malleolar artery curves toward the lateral malleolus and contributes to the periarticular arterial ring at the ankle. Proximally, the anterior tibial artery continues onto the dorsum of the foot as the dorsalis pedis artery, while distal branches arc over the talus and neck of the fibula toward the lateral tarsal region. Superficial dorsal veins form a blue network over the dorsum of the foot, positioned superficial to the arterial trunks and converging proximally toward the anterior ankle. Course and anastomoses at the lateral malleolus matter because skin and soft tissue over the fibula have limited muscular coverage, so small arterial disruptions can translate into wound-healing problems after trauma. Lateral malleolar arterial injury may follow ankle fractures or lateral surgical exposures, and the vessel’s contribution to the malleolar anastomosis becomes relevant when planning incisions or raising local flaps around the ankle. The relationship to dorsalis pedis caliber and the dorsal venous network also supports bedside correlation with pulse palpation and dorsal foot venous access when edema or peripheral arterial disease is in question. Ankle vascular anatomy like this supports teaching in lower limb gross anatomy, podiatry and orthopaedic modules, and surgical anatomy courses focused on approaches to the distal fibula and talocrural joint.
