Seen from the left lateral aspect, the superficial cervical fascia and skin have been lifted to reveal the sternocleidomastoid running obliquely from the mastoid process and lateral superior nuchal line to the manubrium and medial clavicle. Posterior to it, the trapezius forms the posterior border of the lateral neck, while the splenius capitis and semispinalis capitis occupy the posterolateral compartment, spanning from the cervical spinous and transverse processes to the occipital bone. Between the posterior margin of sternocleidomastoid and the anterior border of trapezius, the floor of the posterior triangle is suggested by the levator scapulae and deeper paraspinal muscles overlying the cervical vertebrae. Bony landmarks at the cranial base and upper cervical spine anchor the muscle attachments. Clear layering. A lateral dissection-style view like this matters because most clinically relevant neck landmarks are defined by muscle borders, not by bones, and the sternocleidomastoid is the reference for localizing the carotid sheath, cervical lymph node levels, and the anterior versus posterior triangles. Spasm or shortening of sternocleidomastoid produces congenital or acquired torticollis, and the resulting head tilt and rotation make more sense when you can trace its superior attachment near the mastoid. Posteriorly, tension and strain at the splenius capitis and semispinalis capitis attachments commonly contribute to whiplash-associated pain patterns, and these muscles become key targets during trigger point examination and guided injections.
