Description
Obliquely wedge-shaped, the L5 vertebral body sits inferior to L4 and immediately superior to the base of the sacrum, its anterior height exceeding the posterior height to support the lumbosacral angle. From the lateral aspect you would expect to appreciate the vertebral arch sweeping posteriorly to the spinous process, with the superior and inferior articular processes projecting near the pedicle-lamina junction. The L4–L5 and L5–S1 intervertebral discs occupy the adjacent intervertebral spaces, and the lumbar lordosis is apparent as the column curves anteriorly in this region. Small but telling landmarks, like the mammillary process on the superior articular process, often read well in profile. L5 matters because it is the transition vertebra between the mobile lumbar spine and the fused sacrum, and the geometry you see laterally explains common mechanical problems. Forward translation of L5 on S1 in isthmic spondylolisthesis relates to defects of the pars interarticularis, while degenerative disc disease at L5–S1 alters segmental lordosis and can narrow the lateral recess where the S1 nerve root travels. This is also a key teaching view for correlating sagittal alignment with load transfer across the lumbosacral junction. Use this asset in spine anatomy and biomechanics modules, radiology teaching files when introducing lateral lumbar radiographs and sagittal CT/MR correlations, and in surgical education discussing L5–S1 fusion, reduction of spondylolisthesis, or restoration of lumbar lordosis.
