Description
Seen from the medial aspect of the right posterior thigh, the long head of the biceps femoris occupies the posterolateral hamstring compartment while remaining closely apposed to the semitendinosus along its proximal and mid-thigh course. Superiorly, its tendon arises from the ischial tuberosity of the pelvis and descends posterior to the femoral shaft, with the semimembranosus lying more medially and deep where visible. Distally, the long head converges toward the lateral knee to join the biceps femoris tendon inserting on the head of the fibula, lateral to the tibia and posterior to the patella in relation to the knee joint line. Clear relationships. Appreciating the long head from a medial perspective helps when teaching how the hamstrings partition the posterior thigh and how their tendons diverge around the popliteal region, where small changes in tendon position matter for diagnosis and repair. Proximal hamstring strains and ischial tuberosity avulsions often involve the common origin shared with semitendinosus, while distal biceps femoris injuries and tendinopathy are relevant to lateral knee pain and posterolateral corner mechanics.
